Dear Valued Customer,

All Zee Wholesale customers are required to fill out the PACT ACT Form to ensure that we can continue to ship to your locations!

The PACT ACT requires us to obtain the following documents in order to fulfill/ship any orders past March 27th, 2021:

-Business License (State Sales Tax Permit)

-Federal EIN Document (FEIN)

-Tobacco License or OTP (if required by State)

If you have multiple locations, please include documentation for all addresses where we ship.
